Students are required to submit a new diet prescription form each school year.  If your child requires a special meal due to allergy or disability, please go to your licensed physician to receive an updated diet prescription.  You may download the form from this website in the document Uploads section.  Once your physician has completed this diet prescription form, you may return it to the school nurse at either school.
